Casey Health Institute
The Casey Health Institute (CHI) located in Gaithersburg, Maryland recently opened in May 2013. CHI provides sustainable integrative medicine within a patient-centered medical home model. CHI provides services such as 24/7 access to one's healthcare record, shared medical appointments, health coaches, integrative medicine team rooms in which patient cases are collaboratively discussed by the integrative team, and a wellness center with exercise, nutrition, behavioral and stress reduction classes. CHI is committed to improving access to care for the underserved by providing sliding scale payment options for patients who qualify. In addition, CHI is focused on researching medical and economic outcomes of its practice.
Clinical services: Integrative primary care, acupuncture, chiropractic, massage therapy and healing touch, naturopathy, nutritional counseling and yoga.

Center for Natural Medicine
The Center for Natural Medicine is a premier naturopathic healthcare facility in Portland, Oregon with a group of seven naturopathic physicians that operates as a Level 3 primary care medical home under recognition by the state of Oregon. The Center offers primary care naturopathic medicine and collaborates with other integrative practitioners to deliver quality health care.
Clinical services: Naturopathic medicine, acupuncture and Oriental medicine, massage therapy.
Clinix Healing Center
The Clinix Healing Center located in Centennial, Colorado is a designated patient-centered medical home providing integrative family medicine care. The Clinix healing center achieved as a PCMH in 2009 and two practitioners have recently received recognition by NCQA/American Diabetes Association Physician program for July 2014 - July 2017. Clinix has a team of M.D.s, D.O.s, chiropractors, and massage therapists offering a holistic integrative experience for their patients. Clinix utilizes a patient portal allowing patients to track their own health progress and contact their physicians directly.
Clinical services: Family medicine, preventative wellness visit, exercise therapy, nutria-scripts, chiropractic, massage therapy, hormone therapy, detoxification program, allergy elimination therapy, skin and aging, and microcurrent therapy.
Healthpoint's 13 clinics in Washington State are the first publicly funded natural medicine centers in the nation. Healthpoint prides itself in being a "patient-centered healthcare home" which not only provides medical services, but also includes dental, naturopathic, nutritional, and acupuncture services. The National Committee for Quality Assurance (NCQA) recognizes Health point as a level 3 Patient Centered Medical Home, the highest level possible. Healthpoint provides healthcare to the underserved and focuses on educating future integrative physicians with its partnership with A.T. Still University.

Established in 2008, Heartsong is the only non-profit naturopathic medical clinic providing primary care in the state of Vermont. The very small clinic was recently recognized in 2013 as a Level III Advanced Primary Care Patient-Centered Medical Home as a result of exemplary use of electronic health records, coordinated care and patient self-management. Heartsong's philosophy is centered on integral health, equitable access, community resilience, health promotion and sustainable medicine. The clinic promotes healing partnerships between patients and providers and creates personal treatment plans unique to each patient and is a learning environment dedicated to continual improvement.

Lane County Health Center
Lane County Health Center (LCHC) of Oregon is a federally qualified health center (FQHC), which has adopted a patient-centered medical home model to improve the health and wellness of Lane County through access to affordable, holistic healthcare. LCHC was the first county health department to hire a naturopathic physician in 2013. LCHC coordinates patients' health care across the community, among specialists, emergency and hospital visits, diagnostic testing services, and other services. Patients are assigned to specific integrative teams that manage and coordinate their care.
Clinical services: Prenatal care, family planning, naturopathic medicine, pediatrics, family practice and internal medicine, integrated behavioral health, low-cost medications program, dental prevention program.
Manakai O Malama
The Manakai O Malama integrative healthcare group and rehabilitation center is located in Honolulu, Hawaii. This unique facility blends modern and traditional healing practices with a patient-centered approach. Inspired by the principles of integrative medicine, the goal is to provide whole person care that balances patients' physical, social, emotional, and spiritual factors. The clinic employs a variety of practitioners including acupuncturists, psychologists, occupational therapists, massage therapists, chiropractors, and medical doctors. Lifestyle wellness is a prominent feature of Manaki O Malama and emphasizes diabetes education, weight management, and chronic pain. With the addition of the Niolopua Sleep Wellness Center, the center also values and promotes sleep wellness through comprehensive multidisciplinary services to achieve optimal sleep.

Mountain View Natural Medicine
The Mountain View Natural Medicine Clinic located in Burlington, Vermont is situated within a larger, integrative health center, Eastern View Integrative Medicine, and achieved PCMH status in 2013. The focus of the clinic is whole patient care and partners with each patient to treat and prevent health issues. The clinic is led by a team of naturopathic physicians and offers services including general practice, women's health, complementary medicine, preventative medicine and supportive care through community health workers.

Old Town Clinic
Old Town Clinic is a multifaceted integrative primary care clinic serving individuals impacted by homelessness, poverty and addictions. The clinic is uniquely structured with several designations; it is a Healthcare for the Homeless Clinic, an FQHC, a Patient-Centered Primary Care Home by Oregon's standards, and lastly, it is a PCMH with level 3 NCQA certification. Old Town Clinic utilizes a team-based model of care comprised of naturopathic doctors, medical doctors, physician assistants, nurse practitioners, behavioral health providers, social workers, care team managers, and other assistants. The approach at Old Town Clinic is an amalgamation of both naturopathic and allopathic medicine emphasizing whole person care. The clinic offers a variety of group services and weaves in integrative therapies whenever possible. As an example, the pain management group incorporates various mind-body techniques including breathing, meditation, and positional therapy. In addition, acupuncture is yet another tool the team relies on for treating pain and addictions. In 2012, the clinic was selected to be part of a study sponsored by the Robert Wood Johnson Foundation titled, The Primary Care Team: Learning from Effective Ambulatory Practices, which identified Old Town Clinic as one of the 30 sites across the country demonstrating innovative concepts in primary care.

- Southcentral Foundation
The health system of Southcentral Alaska is called the Nuka System of Care and is managed by Alaska Native people for Alaskan Native people. It provides and coordinates all aspects of healthcare, addressing physical, mental, emotional, and spiritual wellness. The foundation has an online portal allowing patients to track their medical records across different providers.
Clinical services: Primary care clinics, obstetrics and gynecology, behavioral health, dental services, pediatrics, optometry, elder program, health and wellness program, traditional healing, native men's wellness program, emergency department, pharmacy, audiology, acupuncture, chiropractic, massage, and home-based services. Traditional services: healing hands and massage, culturally-sensitive supportive counseling and storytelling, cleansing, healing touch, talking circles, prayer, songs, dances, consultations with elders, traditional healing garden.
Venice Family Clinic
The Venice Family clinic provides healthcare to individuals in need and emphasizes the needs of women, children, the homeless and those with chronic diseases. The clinic has an impressive community spirit since nearly 2,100 practitioners donate their time annually and also collaborates and receives support from UCLA Medical Center, Santa Monica-UCLA Medical Center and Orthopaedic Hospital, Saint John's Health Center, Kaiser-Permanente, and Cedars-Sinai Medical Center. The Venice Family Clinic offers primary care, specialty care, mental health, dental and health education. Through the Simms/Mann Health and Wellness Center, the clinic offers a variety of evidence-based integrative services focusing on the whole person and whole family. The Simms/Mann Clinic is the nation's first health, wellness, and integrative medicine program to be offered at a community clinic that specializes in chronic pain and stress management.
